You complete the online referral form
We contact you for intake
We do the initial assessment
Assessment report completed within 2 weeks
We ask that you take the time to complete the referral form with as much detail as possible. After reviewing your referral form, we’ll contact you via phone to go over our services and get any clarifying information.
If we’re a good fit, we’ll send paperwork to your support coordinator and get an initial assessment scheduled
